- Immunization record
- TB test or Risk Assessment
- Proof of Income from all sources, employment, disability, unemployment, TANF,
- Child Support or other income. Items must be dated for the month prior to enrollment
- Proof of Residence (2 utility bills) - In Lieu of bills see pre-enrollment application
